Why Foundation Week Matters

 Foundation Week gives families a starting point. Through individualized assessments, family collaboration, and strategic planning, we help participants begin building the skills, confidence, and support systems needed for greater independence and future success.

Our goal is to provide clarity, direction, and actionable next steps tailored to each participant’s unique strengths and needs.

What Happens During Foundation Week

 During Foundation Week, participants and families engage in a personalized experience focused on assessment, collaboration, and goal development.

This process includes:

  • Individualized assessments 
  • Family meetings and collaboration 
  • Goal setting and planning 
  • Independence readiness review 
  • Personalized action plan creation 
  • Transition support recommendations 

Each experience is tailored to meet the participant where they are while creating a roadmap for future growth.

Each Participant Receives

 At the completion of Foundation Week, each participant receives a personalized Completion Binder that includes:

  • Individualized Action Plan 
  • Strengths & Needs Summary 
  • Goals & Recommended Next Steps 
  • Resource Guide 
  • Progress Tools 
  • Certificate of Completion 

Families leave with a tangible roadmap they can immediately begin using at home and in the community.
